Every generation of schoolkids retells her story a little differently, but the ending is always the same: she asks if she’s pretty, and there’s no safe answer. Enough retellings over enough decades gave the story a body. She’s faster than she looks and a great deal less patient than the legend suggests.

Traits
Cursed Manifestation. Only creatures that can sense cursed energy can perceive or harm her. She is exorcised when reduced to 0 hit points.
Reinforcement. While she has cursed energy remaining, Kuchisake-onna adds +3 to her AC, saving throws, and damage rolls.
Am I Pretty? As a bonus action, Kuchisake-onna can ask a question of one creature within 30 feet that can hear her. The target must succeed on a DC 17 Wisdom saving throw or be charmed until the end of its next turn. Whether or not the save succeeds, Kuchisake-onna has advantage on the next attack roll she makes against that creature before the end of her next turn.
Actions
Multiattack. Kuchisake-onna makes two Shears attacks.
Shears. Melee Weapon Attack: +9 to hit, reach 5 ft., one target. Hit: 16 (2d10 + 6) slashing damage, and the target must succeed on a DC 17 Constitution saving throw or have disadvantage on Charisma checks that require speaking until it finishes a short or long rest.
Fleetfoot Dash (Recharge 5-6). Kuchisake-onna moves up to twice her speed without provoking opportunity attacks and can make one Shears attack at any point during the movement.
